I've tried a variety of teeth-grinding guards, and this is the only one I have been able to tolerate so far. I've tried the type you heat and custom-mold, but never could get past the actual molding process, due to temperature-related pain and general hassle. I've also tried the newer models that just have a thin soft strip that falls across your lower teeth with two adjustable bite plates, but that one scares the hell out of me as a possible choking hazard, since it doesn't stay in place.
I've used this model for years now. I have TMJ and just can't stop the nighttime teeth-grinding. There's no way to get around the fact that you'd rather NOT have to wear this, but compared to other products out there, it's a good combination of safe (no choking hazard) and reasonably comfortable (not as bulky as other models).
They recommend you wear this on your lower teeth, but for me it's more comfortable to use on my uppers. It's size-customizable in the sense that when you get it, you can trim it down with (very sharp) scissors. It's helpful to have an emery board on hand to soften up any rough areas after trimming. Take some time with this and get it just right. The plastic it's made from is semi-hard, but also malleable over time. The pressure from your teeth will cause it to indent to your bite. It's alarming, actually, when you start to see how hard you're actually grinding your jaws.
As for the bulk, it does push your lip out some, but not as bad as, say, a sports guard. You also can not speak quite normally while it's in, which can be a good thing if you talk in your sleep and don't want anyone to be able to make out what you say. ;) It takes a few days to get accustomed to. You'll spit it out the first few nights. If you mouth-breathe, it will bother you at first, but you CAN breathe with it in, so you'll probably adjust.
As to its durability, I find I have to replace it about once every 8 months or so, and not because it actually falls apart or anything. It just gets mangled after a while. (Better than it being your fillings!)
Bottom line, if you're like me and the other products out there haven't worked for you because they're too much hassle, or make you afraid of choking, this might be a good middle-ground. It has helped relieve my TMJ-related pain and protected my teeth. The price is very fair, too.
A note to the manufacturer: if you could make that section that goes across the front teeth a little thinner, to prevent it from pushing out the lip, this would be close to perfect.
